29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a.) Greenline Foods, Inc. located at 12700 S. Dixie Highway Bowling Green, OH; The employer did not assure employees were protected from caught by hazards while cleaning around the conveyor system. On February 18, 2012 an employee received a broken forearm when cleaning the green beans from under Conveyor 159.    b.) Greenline Foods, Inc. located at 12700 S. Dixie Highway Bowling Green, OH; On or about April 4, 2012, the employer did not assure employees were protected from a rotating shaft on Snipper machines such as but not limited to; Snipper #108. Employees are required to clean out the green beans from under the #108 Snipper exposing them to caught by hazards from a rotating shaft.
