One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Press Area: Rollers located at the top of the Miehle Offset Press (UV fountain area) were not guarded exposing employees to the ingoing nip points, as observed on or about March 23, 2012. b) Press Area: Employees were exposed to a shear point between the gap of the plate roller and the nip point guard on the Heidelberg die cutting press, as observed on or about March 23, 2012.
