29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. a) 1862 Aloha Lane - On or about April 3, 2012, a trench over 5 feet deep had the spoil pile lying at the edge of the trench creating a potential cave in hazard to employees laying GeoThermal pipes. b) 1862 Aloha Lane - On or about April 3, 2012, a trench over 5 feet deep had a ladder jack scaffold and wood material lying less than 2 feet back from the edge of the trench causing a cave in hazard to employees laying Geo Thermal pipes.
