29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      On or about April 4, 2012 and times prior thereto, employees of M&M Manufacturing, Inc. operated unguarded machinery for the fabrication of metal ductwork.  The machinery observed was not guarded at the point of operation, exposing employees to hazards such as amputations.  The machinery observed was as follows:    a)  Employees operated an unguarded riveter machine (SN#:912-9005-SS, Part #: 6001) manufactured by Chicago Rivet and Machine Co.  The riveter machine had a 1  inch opening between the jaws and anvil at the point of operation.    b)  Employees operated an unguarded riveter machine (SN#912-9005-SS, Part#5001) manufactured by Chicago Rivet and Machine Co.  The riveter machine had a 1  inch opening between the jaws and the anvil at the point of operation.      c)  Employees operated an unguarded riveter machine (SN#11824, Model#912) manufactured by Chicago Rivet and Machine Co.  The riveter machine had a 1  inch opening between the jaws and the anvil at the point of operation.    d)  Employees operated an unguarded riveter machine (SN#912-7907-SS, Part#3479) manufactured by Chicago Rivet and Machine Co.  The riveter machine had a 1  inch opening between the jaws and the anvil at the point of operation.    e)  Employees operated an unguarded riveter machine (SN#912-3519) manufactured by Chicago Rivet and Machine Co.   The riveter machine had a 1  inch opening between the jaws and the anvil at the point of operation.    f)  Employees operated an unguarded riveter machine (SN#912-7908-SS, Part#3479) manufactured by Chicago Rivet and Machine Co.  The riveter machine had a 1  inch opening between the jaws and the anvil at the point of operation.    g)  Employees operated an unguarded riveter machine (SN#912-5910) manufactured by Chicago Rivet and Machine Co.  The riveter machine had a 1  inch opening between the jaws and the anvil at the point of operation.    h)  Employees operated an unguarded riveter machine (SN#11702-SS, Model#912) manufactured by Chicago Rivet and Machine Co.  The riveter machine had a 1  inch opening between the jaws and the anvil at the point of operation.    i)  Employees operated an unguarded metal former stamp-lok machine (SN#3292-600) manufactured by Maplewood.  The stamp-lok  had a 1 inch opening at the point of operation.    j)  An unguarded bantam press brake (SN#PBU/4/503, Model#B412).  The bantam press brake was operated by a foot pedal and had an opening of approximately 1 inch at the point of operation.    k)  An unguarded shearing machine (Model#5F) manufactured by Niagra Machine and Tool Works.  The shears had an approximate 1 inch opening at the point of operation.    l)  An unguarded mini-press (No SN# or Model# available) manufactured by M&M Manufacturing Company.	The mini-press had a  inch opening between the press and the table.    m)  An unguarded drill press (No SN# or Model# available) manufactured by South Bend Lathe Works.  The drill bit used in the drill press was measured at 4 inches.
