29 CFR 1910.243(c)(3): Abrasive wheels used on vertical portable grinder(s) (right angle head grinders) were not provided with safety guard(s) having a maximum exposure angle of 180 degrees and located between the operator and wheel: At 10355 E. Linwood Ave., Shreveport, LA 71106; the employer did not ensure that each portable grinder was properly guarded. This condition was observed on or about March 20, 2012 where the abrasive wheel guards on Metabo portable grinders (Part No. 98580 / 11520), being used in the shop area, did not cover the entire grinding wheel allowing the operating wheel to extend beyond the installed guard. Employees grinding various metal pieces were exposed to amputation, laceration and/or struck-by hazards.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): At 10355 E. Linwood Ave., Shreveport, LA 71106; the employer did not ensure that each electrical extension cord was visually inspected before being used. This condition existed on or about March 20, 2012 where several extension cords for abrasive grinders and other equipment being used by welders in the shop area were frayed and spliced with damage to the outer jacket/insulation, exposing the employees to electrical shock hazards.

Linwood Ave., Shreveport, LA 71106; the employer did not ensure that all portable fire extinguishers were inspected and tested. This condition was observed on or about March 20, 2012 in the storage area and throughout the site where portable fire extinguishers had not received an annual maintenance inspection since year November 2006 and were not visually inspected by the employer at least monthly.
