(a) On or about 03/30/2012 at 18 Adams Court, Monroe, NJ 08831: Employees using the DeWalt D55270 5.5 HP Gas 8 Gal. Wheeled Portable Compressor were exposed to the moving flywheel that was not protected by a guard.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper surface to which the ladder was used to gain access.    (a) On or about 03/30/2012 at 18 Adams Court, Monroe, NJ 08831: The ladder provided did not extend 3 feet above the working surface of the roof and was not secured to allow for safe access on and off the roof. Employees using this ladder were exposed to falls of approximately 21' to the ground below.

29 CFR 1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identified them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:    (a) On or about 03/30/2012 at 18 Adams Court, Monroe, NJ 08831: The Louisville 32' Extension Ladder used to access the roof had damaged footpads which were bent inward located at the base of the ladder. Employees accessing this ladder were exposed to fall hazards of approximately 21' to the ground below.
