29 CFR 1926.451(b)(5)(ii): Each platform greater than 10 feet in length shall not extend over its support more than 18inches (46 cm), unless it is designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, orhas guardrails which block employee access to the cantilevered end. a. On or about April 4, 2012 - at the above addressed jobsite, employees were performing siding installation from a ladder jack scaffold that had the platform extending greater than eighteen (>18") inches past its supports, thereby exposing employees to falls of eleven (11) feet five (5) inches and seventeen (17) feet seven (7) inches.

On or about April 4, 2012 - at the above addressed jobsite, employees conducting siding installation from a ladder jack scaffold, eleven (11) feet five (5) inches and seventeen (17) feet seven (7) inches above ground, were not protected by the use of a personal fall arrest systems, thereby exposing the employee to fall hazards.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portableladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps,walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall beused. Crossbraces shall not be used as a means of access. a. On or about April 4, 2012 - at the above addressed jobsite, employees conducting siding installation from a ladder jack scaffold, eleven (11) feet five inches (5) and seventeen (17) feet seven (7) inches above ground, were not provided with an access ladder to the scaffold platform, thereby exposing employees to the hazards associated with falls.
