29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers was not equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails: Employees were exposed to a fall hazard from the walkway along the drillers side of the rig carrier at a four step stairway where the stairway handrail was missing: 1. On the right side of the stairway at the entrance to the walkway there was a nineteen and one-half inch wide opening. 2. On the left side of the stairway at the entrance to the walkway there was a seveteen and one-half inch wide opening. 3. At the vee door of the elevated rig floor a nine step stairway which lacked mid rails on both sides.

OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a burn/fall hazard. At this inspection site, on March 30, 2012, and at times prior, the auxiliary escape line was installed in an unsafe manner and included the following deficiencies: a. A securely anchored one-half inch wire rope Geronimo Emergency Escape line was attached to derrick or mast so as not to provide a ready and convenient means of egress from the Derrickman's working platform. b. The Geronimo Emergency Escape Device was installed on the wire rope escape line which was secured above the center of an opening in the guardrails of the pipe racking board on the southeast side. c. The Geronimo Emergency Escape Device was installed on the wire rope escape line and secured at an angle and up against the edge of the guardrails. d. The one-half inch wire rope Geronimo Emergency Escape line was stretched out. e. The one-half inch wire rope Geronimo Emergency Escape line has interior wires protruding through exterior lays of the wire rope. f. The one-half inch wire rope Geronimo Emergency Escape line was lacking lubrication. g. The end of the wire rope Geronimo Emergency Escape line had been laid around the base of a steel stake. h. The wire rope Geronimo Emergency Escape line after being laid around the base of a steel stake only had two Crosby clips attached to the one-half inch wire rope and they were improperly installed. The C. C. Forbes Company, L. P. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 311065874, citation number 1. item number 1 and was affirmed as a final order on November 15, 2007 with a final abatement date of December 19, 2007, with respect to a workplace located at Clara Driscoll A-25 FM Hwy 665, Driscoll, Tx 78371.
