29 CFR 1926.102(a)(1):     Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:      a) On or about 04/04/12 at 14 Cleveland Avenue, Milltown, NJ 08850 an employee was installing roof shingles using a pneumatic nail while not wearing any eye and or face protection.

29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above the lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of 29 CFR 1926.502(k) developed and implemented:    a) On or about 04/04/12 at 14 Cleveland Avenue, Milltown, NJ 08850 four employees were installing new roof shingles on an 8/12 pitch roof, an eave height of 21'6" to ground, with no fall protection system in place.
