29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. (a) Mejiya Construction, 209 Olympia Ave., Longmont, CO 80504: On and before May 5, 2012, the exposing and controling employer did not provide fall protection for employees performing framing work on the second floor of a two story single family home near an opening for a staircase (which had not yet been installed) approximately 15 feet in length and 3 feet in width. Opening exposed employees to fall hazards of approximately 9 feet to the first floor and then continued uninterupted to the basement floor for a total fall height of 19 feet.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed inorder to minimize these hazards. (a) Mejiya Construction, 209 Olympia Ave., Longmont, CO 80504: On and before May 5, 2012, the exposing and controling employer did not provide fall protection training program for employees performing framing work on the second floor of a two story single family home near an opening for a staircase (which had not yet been installed) approximately 15 feet in length and 3 feet in width. Lack of training resulted in employees not being able to recognize the fall hazzard raised by the staircase opening as they were raising walls to farme the second floor of the work site.
