29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.     a)  Kustom Dream Home, Inc. on or about April 04, 2012, at the address of 5855 Hagman Rd. Toledo, OH: The employer did not ensure employees were protected from falls while performing roofing work on a low sloped commercial structure. Employees installing flashing material atop the roof structure were exposed to a 40 fall to the ground below.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    a)  Kustom Dream Home, Inc. on or about April 04, 2012, at the address of 5855 Hagman Rd. Toledo, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems, guardrails or alternative acceptable measures while performing roofing operations on a low sloped roof.
