OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed the the hazard of being struck by debris while working on or around pressurized vessels. For the employees conducting hydrostatic testing on oil field equipment on or about April 3, 2012, the employer did not ensure employees were provided with adequate written procedures to safely conduct pressurization, testing, and depressurization of equipment.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees are exposed to the hazard of explosion and being struck by when working on or around pressurized equipment. For the employees conducting hydrostatic pressure testing on or about April 3, 2012, the employer did not ensure gauges were directly on the equipment being tested to verify the equipment contained pressure.
