29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.      A)  191 County Road, Creskill - NJ: An employee was exposed to fall hazards of approximately 13 feet while engaged in glass setting work while working from a roof, on or about February 7th 2012.

29 CFR 1926.453(b)(2)(v):     A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:      A)  191 County Road, Creskill - NJ: An employee was using an aerial lift to access a 13 foot roof and was not protected from falls by a personal fall arrest system, on or about February 7th 2012.      City of Newark Glass --- was previously cited for a violation of this Occupational Safety and Health Standard or it's equivalent 29 CFR 1926.453 (b) (2) (V) which was contained in OSHA inspection # 315153379, Citation 01, Item 002, issued on 04/08/2011 with respect to a workplace located at 444 Ocean Avenue in Long Branch, NJ.
