29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points: a) 994 Madison Avenue, Basement- On or about 4 April, 2012 - Energized temporary lighting wiring was pinched between the adjoined metal wall studs as it passed from one room into the hallway. Employees were exposed to a potential shock and fire hazard.

29 CFR 1926.405(g)(2)(iii): The repair splice in a hard service flexible cord No. 12 or larger did not retain the insulation, outer sheath properties, and usage characteristics of the cord which was being used: a) 994 Madison Avenue, 2nd floor, on or about 4 April 2012- Three separate temporary wire conductors were spliced together with wire nuts. Outer sheath properties were not maintained. In the event the wires were pulled apart employees would be exposed to an energized conductor and an electrocution hazard.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) 994 Madison Avenue, Second floor, on or about 4 April 2012- Three separate temporary wire conductors were spliced together with wire nuts, hanging in the middle of the room. Splice did not have any strain relief. In the event the wires were pulled apart employees would be exposed to an energized conductor and an electrocution hazard.

Employees were exposed to a fall/tripping hazard. b) 994 Madison Avenue, Basement, first room on the left, on or about 4 April 2012- Temporary electrical wiring was blocking clear access to the hallway. Employees were exposed to a fall hazard.
