29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    Redi-Red Enterprises, Inc., conducting work at 4103 Lagrange St., Toledo, OH: On or about April 5, 2012, the employer did not ensure that employees were protected from fall hazards while performing roofing work on a low-slope pitch roof of the commercial building. Employees were exposed to fall hazards of approximately 18 feet from the roof to the ground below.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Redi-Red Enterprises, Inc., conducting work at 4103 Lagrange St., Toledo, OH: On or about April 5, 2012, the employer failed to ensure that employees had been trained in the appropriate use of fall protection such as, but not limited to, the use of personal fall arrest systems, or alternative measures of fall protection while performing roofing work on a low-slope pitch roof of the commercial building. Employees were exposed to fall hazards of approximately 18 feet from the roof to the ground below.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladders' length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    Redi-Red Enterprises, Inc., conducting work at 4103 Lagrange St., Toledo, OH: On or about April 5, 2012, the employer did not ensure that extension ladders, used to access the roof of the commercial building, were extended at least 3 feet above the roof edge. The employees were exposed to fall hazards of approximately 18 feet when using the ladders to access the working level in order to conduct roofing activities.    No additional abatement information is needed for this item.
