29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):      a) North side of building, on or about 3-30-12:  Two employees were working on a scaffold platform that was not fully planked, exposing the employee to a fall hazard of 12 feet 6 inches.

29 CFR 1926.451(b)(3):     The front edge of all platforms was not 14 inches (36 cm) or less from the face of the work, and no guardrail systems were erected along the front edge and/or no personal fall arrest systems were used in accordance with paragraph (g) of this section to protect employees from falling:      a) North side of building, on or about 3-30-12:  One employee stepped off a roof surface onto a scaffold platform that was 28 inches away.
