29 CFR 1904.32(a)(1): A review of the OSHA Log of Recordable Work-Related Injuries and Illnesses (OSHA Form 300 or equivalent) was not completed to ensure the Log was accurate and complete, and/or deficiencies were not identified and corrected at the end of each calendar year. The employer did not ensure that the OSHA 300 and 300A were accurate and complete for calendar years 2009, 2010 and 2011 as follows: 1.) Column M was not completed for injury numbers 1 and 2 on form 300 for calendar year 2009. 2.) Columns J-G were not completed for injury numbers 2-4 on form 300 for calendar year 2010. 3.) Field K on form 300A did not match Column K on form 300 for calendar year 2010. 4.) Column I was inaccurately checked for injury numbers 1, 3, and 4 on form 300 for calendar year 2011. 5.) Number of Cases field I on form 300A for calendar year 2011 was inaccurate.
