29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.      (a) Childcare Center:  On or about APR 4, 2012 and at times prior thereto, the electrical extension cord to the portable power tools such as BOSCH  Hammder Drill used by the workers was  not equipped with a ground fault circuit interrupter (GFCI) exposing the workers to electric shock and severe burns.

29 CFR 1910.1200(h)(1):  Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59)      (a) Childcare Center:  On or about  APR 4, 2012 and at times prior thereto, information and training was not provided for hazardous materials such as, but not limited to aerosol spray cans (blue cans) that contains acetone, MEK and propane.
