29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.      (a) Seed Lab-OIP Building:  On or about 05 APR 12 and at times prior thereto, two workers on the roof were provided with personal fall arrest systems but only one was tie off and therefore the other worker was exposed to a fall hazards of about 10 feet to the ground. Both workers came down as soon as I arrived at the jobsite.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:      (a) Seed Lab-OIP Building:  On or about 05 APR 12 and at times prior thereto, one worker was exposed to an impalement hazard while using a step ladder 3 feet above ground adjacent to an 18 inch protruding reinforcing steel.

29 CFR 1926.150(a)(4): Firefighting equipment was not periodically inspected:      (a) Seed Lab-OIP Building:  On or about 05 APR 12 and at times prior thereto, the fire extinguisher had not received an annual maintenance check or inspection. The last inspection shown on the tag was in June 2008.
