(a) Seed Lab-OIP Building:  On or about 05 APR 12 and at times prior thereto, two workers on the roof were provided with personal fall arrest systems but only one was tie off and therefore the other worker was exposed to a fall hazards of about 10 feet to the ground. Both workers came down as soon as I arrived at the jobsite.
