29 CFR 1926.451(b)(10): Scaffold components manufactured by different manufacturers were intermixed such that the components fit together with force and the scaffold's structural integrity was not maintained by the user: (a) On or about 04/06/2012 at 15 Orta Court, Sayreville, NJ 08871: Employee used a TGI Wood Floor Joist Board as a work platform on a Alum-A-Pole Pump Jack Scaffold System. The contractor mixed the components and used materials that were not approved to be used with the Pump Jack System exposing employees to fall hazards of approximately 23' to the ground below.

(a) On or about 04/06/2012 at 15 Orta Court, Sayreville, NJ 08871: Employee installing viynl siding at heights of approximately 23' from the ground below on a pump jack scaffold platform was not provided with a fall protection system. The pump jack scaffold work platform was missing mid and end rails.

(a) On or about 04/06/2012 at 15 Orta Court, Sayreville, NJ 08871: Employee was observed installing vinyl siding from a 6' Werner A-Frame Step Ladder in the closed position resting on a 2X4 wood cleat on a 10/12 pitched roof exposed to fall hazards of approximately 23' to the ground below. The A-Frame step ladder is designed to be used on a flat surface and in the open position with a foot pad touching the work surface.

VMC Construction Inc., was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection #311922611, Citation #1, Item #1, issued on 11/17/2009 with respect to a work place located at Lehigh Avenue, Perth Amboy, NJ 08861 and became a final order on 01/15/2010.
