29 CFR 1926.451(g)(4)(i):     Guardrail systems were not installed along all open sides and ends of platforms.    a) 430 Linfield-Trappe Road, Worksite, Southwest Scaffold  On or about March 30, 2012, employees are conducting masonry work on scaffold with missing guardrails, at least 26 feet above grade, that incorporated inadequate means of fall protection.    No abatement certification or documentation is required.

29 CFR 1926.454(a):     The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    a) 430 Linfield-Trappe Road, Worksite  On or about March 30, 2012, employees are conducting masonry work on tubular welded frame scaffold, with a number of highly visible defects, but had not received training to enable them to recognize these hazards to prevent injury.      Abatement certification required within 10 days after abatement date.  The certification shall include a statement that the abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1926.451(h)(2)(ii):     A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below:    a) 430 Linfield-Trappe Road, Worksite, Southwest Scaffold  On or about March 30, 2012, employees are conducting masonry work on an improperly erected scaffold, at least 26 feet above grade, that was additionally loaded with concrete masonry units that were not restrained from falling to grade by standard toeboards.    Abatement certification required 10 after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.1052(b)(1):     Foot traffic was not prohibited on stairways where the treads and/or landings of pan stairs were to be filled in with concrete or other material at a later date and stairs were not temporarily fitted with wood or other solid material at least to the top edge of each pan.      a) 430 Linfield-Trappe Road, Worksite, West Stairwell  On or about March 30, 2012, workers are accessing the 4th floor using a steel stairwell that does not have all stair pans or the intermediate landing filled with concrete, wood or other solid material.    Abatement certification required within 10 days after abatement date.
