29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury were not guarded. Guarding devices were not in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, were not so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. Location: Building #2, Manufacturing Area The point of operation on the head slitter on the Bulk slitter was not equipped with point of operation safeguarding preventing employees from entering the danger zone created by the ingoing nip points on the head slitter.
