29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: Shop Area: On, or about, March 15, 2012, an employee stood on a pallet and was lifted up to a buss head on the forks of a forktruck. The buss head fed a Do-All vertical band saw and the employee was trying to determine why the machine would not start.

29 CFR 1910.36(b)(1): At least two exit routes were not available in a workplace to permit prompt evacuation of employees and other building occupants during an emergency, except as allowed in paragraph (b)(3) of this section: Third Floor: One exit route was provided from the third floor of the building. A door that led to the unused portion of the building was routinely padlocked shut and no known, properly maintained exits were provided in that section of the building. Two doors located on the exterior walls were nailed shut and covered with plastic. One was totally blocked in by pallets of stored material. The one exit route that was provided was locked on the second floor landing to prevent movement from the third floor to the second floor.

Central Stairway: The exit doors provided in the stairwell were routinely locked with metal latching devices. B. Second Floor: An door labeled as an exit from the second floor, across from the office, was padlocked shut.
