29 CFR 1910.119(d)(3)(ii):     The employer shall document that equipment complies with recognized and generally accepted good engineering practices.  On or about March 20, 2012, the employer did not document that its equipment complies with recognized and generally accepted good engineering practice according to the ASHRAE-15 and IIAR-2 ANSI Standards, or equivalent. Examples include, but are not limited to:  a. Inside and outdside the "North", "S & T" and "Y & Z" engine rooms there were no visual and audible ammonia alarms. Also the meaning of each alarm were not marked by signs near the annunciators.  b. Ammonia alarms shall not be capable of being reset from the outside of the "North" and "Y & Z engine rooms.    c. The "S & T" and "Y & Z engine room doors did not open outward and were not tight fitting, permitting passage of escaping ammonia to other work areas.    d. Piping passing through the interior walls of the "North" engine room was not tightly sealed to the walls through which it passes, permitting the escape of ammonia to other work areas.  e. On the "North", "S & T" and "Y & Z" engine rooms there were no legible and securely attached signs at each entrance reading "Engine Room - Authorized Personnel Only", and further communicating that entry is forbidden except by those personnel trained in emergency procedures when the ammonia alarm has been activated. Emergency shut-down procedures including precautions to be observed in case of breakdown, or leak, were not posted outside of each engine room, immediately adjacent to each door.  f. In the "North", "S & T and "Y & Z" engine rooms ammonia piping was not labeled, did not identify the physical state, the relative pressure of the refrigerant, and the direction of flow.  g. In the "North", "S & T" and "Y & Z" engine rooms the vessels containing ammonia were not marked with information that included the identity of the hazardous chemical contained therein and the applicable hazard warnings for it.  h. The "North", "S & T", and "Y & Z" engine rooms were not provided body showers for use by employee(s). Also eyewash stations were not provided for engine rooms "S & T" and "Y & Z.

29 CFR 1910.119(e)(3)(i):     The process hazard analysis did not address the hazards of the process.  On or about March 20, 2012, the employer did not identify, evaluate and control the hazard to employee(s) occupying the control room office of the "Y & Z" engine room, whose only available evacuation route, in the case of ammonia release, would be through the engine room itself.

29 CFR 1910.119(f)(1)(i)(B):     The employer's written operating procedures covering the steps for each operating phase did not address normal operations.  On or about March 20, 2012, detailed operating procedures suitable for each drain point were not available for oil draining operations, including safety and personal protective equipment specifications. Examples include, but are not limited to, oil drain points on:  a. The suction receiver vessel and condenser receiver oil trap in the "North" engine room.  b. The suction receiver vessel and Pr-1 and Pr-2 ammonia vessels in the "Y & Z" engine room.   c. The suction receiver vessel and inter-cooler vessel trap in the "S & T" engine room.

29 CFR 1910.119(g)(2):     Refresher training shall be provided at least every three years, and more often if necessary, to each employee involved in operating a process to assure that the employee understands and adheres to the current operating procedures of the process. The employer, in consultation with the employees involved in operating the process, shall determine the appropriate frequency of refresher training.  On or about March 20, 2012, the employer did not provide refresher training at least every three years to employee(s) operating its refrigeration process.

29 CFR 1910.119(j)(2):     Written procedures. The employer shall establish and implement written procedures to maintain the on-going integrity of process equipment.  On or about March 20, 2012, the employer did not establish and implement written mechanical integrity program procedures for the inspection, testing and preventative maintenance of its ammonia refrigeration process equipment, such as, but not limited to:  a. Compressors RC-1 thru RC-11 in the "S & T" engine room.  b. Cornell pumps #1 and #2 in the "North" engine room.  c. Insulated ammonia pressure vessels and insulated piping throughout the facility.
