29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use: Employees were exposed to a struck-by hazard as a result of improperly stored cylinders. Old welding cylinders were stored outside along the south wall of the main factory and near a portable building. The valves were not protected from damage that could cause the cylinder to suddenly lose pressure and become a flying projectile hazard.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): A. Employees were exposed to an electrical shock hazard as a result of a damaged flexible extension cord. The receptacle box on the electrical assemby was damaged and terminal connections were exposed. It had been wrapped with duct tape to enclose the receptacle box. The extension cord was located near the Millermatic 252 Wire Welder units #5 and #6. B. Employees were exposed to an electrical shock hazard as a result of a damaged flexible extension cord. Tension pull on the cord had resulted in exposed conductors and terminal joint connections. The extension cord was located near the Millermatic 252 Wire Welder unit #6.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and cables were run through holes in walls, ceilings, or floors. A flexible cord was run through a hole in the south wall of the factory and to a portable building located adjacent to the factory. This condition resulted in an electric contact shock hazard to employees should the cord become damaged and the metal wall become energized by the exposed conductor.
