29 CFR 1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: For the employees working on or around equipment with energy sources on or about April 10, 2012 and at times prior thereto, the employer did not ensure the procedures were equipment specific to control all energy sources exposing employees struck by and high temperature hazards. a) The CP Kelco Lockout checklist for the Fermenter Inoc Drop and or Fill Valve (dated December 2011) does not specifically state the intended use of the procedure and does not specifically identify how to isolate steam when using this procedure. b) The CP Kelco Lockout Checklist for Purasiv indicates locking out the Nitrogen Make up which had been removed approximately 10 years prior.
