29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. a) Building - On or about April 10, 2012 - During roof installation two (2) employees were tied off to a single anchor point that was according to manufacturers instruction intended for only one worker and did not afford the minimum 5000 pounds per employee of support. Employees were exposed to a fall hazard of 8 feet.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee was trained by a competent person in the use and operation of personal fall arrest systems: a) Building- On or about April 10, 2012 - During roof installation, employees were installing roof decking on a 6 on 12 pitched roof deck without properly utilizing conventional fall protection, CSHO observed employees working on the roof without safety harnesses connected to a lifeline. Employees were exposed to a fall hazard of 8 feet.
