29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. a) On or about April 10, 2012, employees were exposed to cave in hazards while connecting a sewer pipe in an inadequately supported trench. Speedy Rooter, Inc., was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA inspection 110346 citation number 1 item 1a issued on December 30, 2011, which became a final order on January 10, 2012, with respect to a workplace located at 1615 Culebra Av., Colorado Springs 80907. Abatement Note: Abatement certification and documentation is required for this item (see enclosed "Sample Abatement-Certification Letter").

29 CFR 1926.651(k)(1): Daily inspection of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in a possible cave-ins, indications of failure of protective systems, hazardous atmosphere, or other hazardous conditions: a) On or about April 10, 2012, employees were exposed to injury while working in a trench in that, a competent person did not perform an manual test of the trench prior to start of work and as needed through the day. Speedy Rooter, Inc., was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.651(k)(1) which was contained in OSHA inspection 110346 citation number 1 item 1b issued on December 30, 2011, which became a final order on January 10, 2012, with respect to a workplace located at 1615 Culebra Av., Colorado Springs, CO 80907.
