29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) Tomah Veterans Affairs Medical Center: Building 40 Maintenance Shop, a guard was not provided on the Ellis Variable Speed Drill Press, model 8500, serial number 85D97031.  b) Tomah Veterans Affairs Medical Center: Building 40 Maintenance Shop back room, a guard was not provided on the ACER Drill Press, model 3VS, serial number 9166158.  c) Tomah Veterans Affairs Medical Center: Building 40 Carpenter Shop, a guard was not provided on the Rockwell/Delta Drill Press, model 62-683, serial number 7C56B11D2D.  The U.S. Department of Veterans Affairs was cited for a violation of the same Occupational Safety and Health Standard on 09/23/2010, which was contained in inspection 311517429, citation 1, item 03, with the date 10/14/2010 of final order/settlement, and means of abatement.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:  Tomah Veterans Affairs Medical Center: The employer's energy control procedures did not clearly and specifically outline procedureal steps for isolating Boilers 1, 2 and 3 in the Boiler Building.  a) The procedure identified 14 lockout points and later in the document referenced 19 lockout points.  b) The procedure utilized the same valve names for different valves on different boilers in close proximity to each other.  c) The procedure failed to identify additional lockout points that were necessary for isolation of the boilers.  The U.S. Department of Veterans Affairs was cited for a violation of the same Occupational Safety and Health Standard on 01/12/2012, which was contained in Inspection 92592, citation number 2, item number 04a, with the date 02/03/2012 of final order/settlement, and means of abatement.
