D,E,F, & G, Sweetwater, Texas 79556. Employees were exposed to fall hazards while accessing the roof with a ladder that was not secured to a rigid support.

29 CFR 1926.1053(b)(6): Ladders shall be used only on stable and level surfaces unless secured to prevent accidental displacement: On March 1, 2012, and at times prior thereto, at the job site located at 602 Jack Lambert Drive Apt. D,E,F, & G, Sweetwater, Texas 79556. Employees ascending and descending to and from the double plate using a ladder that was not erected on a flat stable surface. This condition exposed employees to potential fall hazards.

1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which were fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective was not removed from service until repaired. On March 1, 2012, and at times prior thereto, at the job site located at 602 Jack Lambert Drive Apt. D,E,F, & G, Sweetwater, Texas 79556, employees were exposed to electrocution hazards while operating equipment connected to a flexible cord without a grounding pin.
