29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    Location: 130 Graham St., Brooklyn, NY.    a) An employee was not protected from falling while working from a tubular welded scaffold (4th level platform) that was not fully planked and he was not wearing a personal fall arrest system.  The employee was exposed to a fall of approximately 30 ft.; on or about 4/9/12.

Receptacles on a two-wire, single-phase portable or vehicle-mounted generator rated not more than 5kW, where the circuit conductors of the generator are insulated from the generator frame and all other grounded surfaces, need not be protected with ground-fault circuit interrupters.
