29 CFR 1926.150(a)(4): Defective firefighting equipment was not immediately replaced: (a)(LOCATION)(IDENTIFY SPECIFIC OPERATION(S) AND/OR CONDITIONS)(DESCRIBE HAZARD(S) WHERE NECESSARY) Employees at the job site located at 1321 Boyle Avenue, St. Louis MO on 11 April 2012 had a fire extinguisher with them that had not been inspected. The employer purchased the extinguisher used and had never had it inspected. Employees had the extinguisher available to use while they used open flames to heat the tar on the roof. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).

29 CFR 1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b) Note: If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502. Employees at the job site located at 1321 Boyle Avenue, St. Louis, MO on 11 April 2012 did not have a fall protection system in place for the two employees working on the roof.
