29 CFR 1926.451(e)(1)  When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:  1)  At the jobsite, safe access to the ladder jack scaffold used by employee(s) to apply siding to the north gable end of the home was not provided.  Both supporting brackets were facing away from the gable end of the home.  Employee(s) were required to climb over these brackets to gain access and egress from the pick board.

29 CFR 1926.451(g)(1)(i):  Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  1)  At the jobsite,a personal fall arrest system was not in use by two employees who were applying vinyl siding to the north gable end of the home from a ladder jack scaffold.  The fall distance from the east end of the pick board was approximately 15 feet.  The fall distance from the west end of the pick board was approximately 13 feet.
