29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder. (a) 419 South Patricia, Nixa, Missouri: Employees were accessing the roof using the extension ladder which was in front of a dock door opening on the north side of the building and the ladder was not secured to the building.
