29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: The employer does not ensure that energy control procedures are developed, documented and implemented to control hazardous energy when employees perform maintenance, servicing, and/or setup on equipment with multiple sources of energy such as electrical, hydraulic, pneumatic and mechianical energy. The following violations of this occurred on or about March 22, 2012 where employees were exposed to struck-by and caught-in-between hazards when conducting setup and maintenance activities without energy control procedures. This violation was observed on or about March 22, 2012, in the fabrication shop where the junction box on the Vertical Band Saw, RF916b, was not effectively closed in that it was attached with duct tape.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: The employer does not ensure cords were connected to prevent strain relief from joints or terminal screws. This violatoin was observed on or about March 22, 2012, in fabrication shop, where the cord was separated from the strain relief device on the Hypertherm Plama Cutter Powermax.
