29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder: a) 60 Route 46 East, Mountain Lakes, NJ. A ladder was being used to gain access into the roof top and did not extend above that roof, exposing employees to the hazard of falling. Violation observed on or about 03/28/2012.
