29 CFR 1910.178(l)(6):     The employer did not certify that each operator has been trained and evaluated as required by paragraph (l) and shall include the name of the operator, the date of training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation:    Employees were operating a forklift and the employer did not provide evidence to show that the required powered industrial training and evaluation had been conducted for the operator. There was no written documentation that included the operators name, date of the training, date of the evaluation and the name of the person conducting the training and/or evaluation.

29 CFR 1926.451(e)(1):   When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral pre-fabricated scaffold access, or direct access from another structure, personnel hoist, or similar surface were not used:  Employees were exposed to fall hazards while working from a frame scaffold because they were climbing the frame of the scaffold to gain access to the working platform. Acceptable means of access on an off the scaffold was not provided for the employee.
