29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. On or about April 4, 2012, employees performing framing work were not protected by a protective helmet while working underneath other employees that were framing at higher levels. The employees were exposed to being struck by objects and/or tools from a height of approximately (8) feet above.

29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents. On or about April 4, 2012, employees performing framing work installing rafters using air nailers were not provided eye and face protection. Employees were exposed to eye and face injuries.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about Novemeber 4, 2012, an employee performing framing work was not protected from fall hazards. The employee was exposed to fall hazards of greater than (8) feet to the ground below.

29 CFR 1926.304(d): Guarding. All portable, power-driven circular saws shall be equipped with guards above and below the base plate or shoe. The upper guard shall cover the saw to the depth of the teeth, except for the minimum arc required to permit the base to be tilted for bevel cuts. The lower guard shall cover the saw to the depth of the teeth, except for the minimum arc required to allow proper retraction and contact with the work. When the tool is withdrawn from the work, the lower guard shall automatically and instantly return to the covering position. On or about April 4, 2012, a carpenter employee was cutting rafters using a Porter Cable and a DeWalt circular saw with the guards blocked exposing the employee to cuts, lacerations and/or amputation hazards.
