29 CFR 1910.23(a)(8)(ii): A floor hole cover of standard strength and construction. While the cover is not in place, the floor hole shall be constantly attended by someone or shall be protected by a removable standard railing. On or about April 10, 2012 and times prior thereto, on the pavement adjacant to the Mud Filter Building a missing floor drain cover presented a tripping hazard for employees who work in the area.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about April 10, 2012 and times prior thereto, employees were exposed to point of operation hazards and rotating parts in the following instances: a. The rotating drums on the front side of the Beloit Paper making machine in the area of the dryers. b. On the west side of the re-winder/slitter below the on-off switch there is employee exposure to the unguarded porton of the slitter knife.
