29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a)  Buecomp Inc. - located in Bloomville, Ohio: On or about April 12, 2012, the employer did not have a lock out program for the control of the hazardous energy, which includes machine specific procedures, training, and periodic inspections, while performing maintenance, servicing, clearing or unjamming on machines such as, but not limited to, the extruders, grinders and hose cutters.  Failure to lock out these components while these tasks are being performed exposes employees to amputation, caught in and shock hazards.

29 CFR 1910.178(l)(1)(i):     The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a)  Buecomp Inc. - located in Bloomville, Ohio: On or about April 12, 2012, the employer did not assure that employees who operate the forklifts completed a training program and received a performance evaluation.  The employees use the lifts to carry products throughout the facility and to load and unload trucks in the shipping area.

Employees were not aware of the PPE requirements, first aid measures, and potential health effects of exposure to these chemicals.
