29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system: On or about April 05, 2012, employees working on the roof were not provided fall protection. The employees were exposed to a 19 feet fall hazard.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about April 05, 2012, employees used an extension ladder to access the roof of an existing building. The side rails of the extension ladder did not extend 3 feet above the landing exposing the employees to a fall hazard.
