29 CFR 1910.147(c)(4)(i):  Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.     The employer had not developed written hazardous enegy control procedures for machines with more than one energy source.  Employees were exposed to hazards associated with the unexpected start-up or re-energization of the following equilpment:        1. Rousselle press #1     2. Rousselle press #2     3. Rousselle press #3     4. Tahoe Thermoform Machine     5. Big Ed in the GDG area.     6. Big Ed in the Plant 2.     7. Boat Thermoformer      8. Hartland Thermoformer     9. DMS CNC machine in Plant 2     10. DMI CNC machine in Plant 2

29 CFR 1910.212(a)(1):  Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    At the establishment, the employer did not provide adequate or no guards on the equipment listed below.  Employees  were exposed to rotating parts and caught in hazards:       (a)  The 80 Ton Rousselle Press #1 was not guarded on the backside and on right side end.     (b)  The Rousselle Press #2 was not gurarded on the backside.     (c)  The Rousselle Press #3 was not guarded on the backside.      (d)  The Big Ed press in the GDG area needed the bottom platen guarded.     (e)  The Big Ed press in Plant 2, need the bottom platen guarded.       (f)  The Stauder Router spindel nut was not guarded at the end of the shaft.      (g)  The Tahoe Thermoform machine needed the Rim Cock guarded.      (h)  The Boat Thermoform in Plant 2 needed the bottom platen guarded.     (i)    The Hartland Thermoform in Plant 2 needed the bottom platen guarded.

29 CFR 1910.219(d)(1):  Guarding. Pulleys, any parts of which are seven (7) feet or less from the floor or working platform, shall be guarded in accordance with the standards specified in paragraphs (m) and (o) of this section.     At the establishment the employer did not provide guards for the Powermatic Saw drive pulley.  Employees were exposed to caught in hazards.

29 CFR 1910.219(e)(1)(i):   Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard shall extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt shall be fully enclosed in accordance with paragraphs (m) and (o) of this section.    At the establishment the employer did not provide guards for the Powermatic Saw drive belts.  Employees were exposed to caught in hazards.
