29 CFR 1910.23(c)(1):     Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):      Employees were exposed to fall hazards of 11 feet, due to lack of fall protection measures such as a personal fall arrest system or fall restraint system while loading carpet material onto forklifts while at the edge of the open sided storage area.  The rack is used to store carpet rolls.

29 CFR 1910.178(l)(6):     The employer did not certify that each operator had been trained and evaluated as required by this paragraph (l):    Employees were exposed to hazards associated with the unsafe operation of powered industrial vehicles because the employer had not certified all operators were competent to run the trucks.  The three authorized employees to operate the forklift had not been certified that they had been trained and evaluated regarding safe powered industrial truck operations.

29 CFR 1910.132(d)(1):     The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):        A hazard assessment was not performed to ensure that employees operating the Cub Cadet 27 HP riding mower were wearing safety eyewear and footwear while using the mower, as the Operator's Manual Safety Precautions mandated.
