29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of anyanchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall bedesigned, installed, and used as follows: The system must be installed under the supervision of a qualified person. a. On the roof of the Northwest Administrative Building where employees were placing plywood on the roof, the anchorage system installed by one of the employees to attach personnel fall protection system was not installed under the supervision of a qualified person thereby exposing employees to a fall hazard.
