29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: A. Lingvai Excavating, LLC worksite located at the intersection of Henry St. & Main St., Montpelier, Ohio: On or about April 11, 2012, the employer did not ensure an employee was adequately protected from cave-ins while installing sanitary lines in an 8.20 feet deep, near vertical trench. Approximately 2.50 feet of the east wall of the trench was unprotected.
