29 CFR 1910.147(c)(4)(i):   Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.     At the job site 12057 West 59th Hwy, Oswego, Kansas, employees were exposed to injury from the unexpected energization of equipment since the employer had not developed written energy control procedures for the following equipment with more that one energy source:         (a)   Air Compressors       (b)   Burn Table       (c)   Wiedemann Cutter       (d)   Cincinnati  Press Brake, 350 Ton       (e)   Piranaha Iron Worker, 110 Ton

29 CFR 1910.212(a)(1):  Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    In the fabrication area, the following machines were not fully guarded exposing employees to amputation and caught in hazards:         (a)  The 350 Ton Cincinnati Press Brake was being operated without the side throat areas being guarded.          (b)   The Century Shear was not guarded on the backside of the shear.

29 CFR 1910.212(a)(3)(ii):  The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    Employee was operating the Piranaha Punch machine without point of operation guarding.  Employee was exposed to amputation and caught-in hazards.

29 CFR 1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch. (See Figures O-18, O-19, O-20, O-21, O-22, and O-23.)    At the job site, employees were using the Tradesman 8" pedestal grinder without a tongue guard.  Employees were exposed to struck by hazards.

29 CFR 1910.304(g)(5):  The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective.    Employees working at the facility were using a Tradesman pedestal grinder with the grounding pin missing from the plug. Employees were exposed to electrical shock hazards.
