The employer did not ensure that electrical equipment was free from recognized hazards that were likely to cause death or serious physical harm to employees: At the job site, the following electrical equipment was not free from recognized hazards: a/ One, yellow flexible cord was placed across the street from the temporary power pole to the home under construction. This cord was been driven over by a variety of motor vehicles and subject to severe crushing.

The narrow end facing the walking/working area measured approximately 3'W X 7'H X 9'D.

Each flight of stairs was seven risers high.
