29 CFR 1926.403(b)(1):     Examination. The employer did not ensure that electrical equipment was free from recognized hazards that were likely to cause death or serious physical harm to employees:      At the job site, the following electrical equipment was not free from recognized hazards:    a/  One, yellow flexible cord was placed across the street from the temporary power pole to the home under construction.  This cord was been driven over by a variety of motor vehicles and subject to severe crushing.

29 CFR 1926.501(b)(4):  Each employee on walking/working surfaces was not protected from falling through holes more than six feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:    a/  At the job site, one floor hole above the basement was inadequately guarded.  The narrow end facing the walking/working area measured approximately 3'W X 7'H X 9'D.

29 CFR 1926.1052(c)(1)(i):  Enclosed stairways having four or more risers or rising more than 30 inches (76 cm), whichever was less, was not equipped with at least one handrail:    a/  At the job site, two flights of wooden stairs leading up to the second story of the home under construction were not equipped with a least one hand rail.  Each flight of stairs was seven risers high.
