29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest system. Employees were exposed to fall hazards of up to approximately 30 feet while performing roofing activities without appropriate fall protection in place.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. Employees were exposed to eye injuries while using a pneumatic nail gun to install shingles and eye protection was not used. The Schaefer Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA Inspection number 310803788, citation number 1, Item 2a and was affirmed as a final order on December 3, 2008, with respect to a workplace located at 9721 Windy Lane Road, Bloomington, IL 61704.
