29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of anyanchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall bedesigned, installed, and used as follows: (a). Worksite located at 503 N. Main, Maryville, Missouri- An employee who was exposed to a fall hazard had attached his rope to an inadequate roof anchor that would not support the force pounds as well as likely slip off of this anchor if the system were to attempt to activate.
