29 CFR 1910.305(b)(1)(ii): Unused openings in cabinets, boxes, and fittings shall be effectively closed. a) Electrical breaker boxes had uncovered circuit breaker openings that exposed employees to possible contact with live electrical conductors and the hazard of electrical shock: 1.) In warehouse breaker panel B had an uncovered circuit breaker opening. 2.) In the storage room the breaker box had 4 uncovered circuit breaker openings.

29 CFR 1910.37(b)(6): Each exit sign must be illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color. Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 footlamberts (0.21 cd/m��?) are permitted. Two exit signs in the warehouse was not illuminated.
